How to Request your Medical Records

If you wish to obtain a copy of your medical record, we are here to assist you.  You must complete the Access PHI Form; you will need a government issued ID to complete the electronic request.  If you are the Parent, Conservator or Legal Guardian of the client you may be asked to submit documentation of your legal authority.  Please click below to start your request.

How to Request my Medical Record be sent to a Third Party?

If you wish to have your Medical Record, sent to a third party, you must complete an authorization form. This form must be completed by the client or their legal representative, (Parent, Legal Guardian or Conservator.) If you are the Parent, Legal Guardian or Conservator of the client you may be asked to provide documentation of your legal authority. All requests require ID for verification. How to Request a Review to Amend your PHI

If you believe that information in your medical record is incorrect or incomplete, you have the right to request an amendment under the Health Insurance Portability and Accountability Act (HIPAA). This process allows you to ensure your health information is accurate, complete, and up to date.

All request for records must be submitted in writing.  You can use our Access PHI form here.  You will be required to provide a copy of your ID when you submit your request.  If you are the legal representative of the client, you will be required to attach a copy of the documentation identifying your legal authority.  We can send records a few ways: MyChart, DocuSign, fax or they can be picked up in person, we do not mail records.  MyChart is the quickest way to access your record, if you do not have a MyChart account and are interested in creating one, please visit our MyChart page for more information.
