Community Support Program/Recovery Pathways Case Management Program

Case Management services provide short-term community advocacy and long- term case management assistance to help consumers lead lives of independence and choice in their community settings. Clients are assisted as they return to the community following hospitalization, and support and discharge planning is provided while clients are still hospitalized. The program helps consumers learn what is needed to successfully manage symptoms and obtain entitlements, education, and employment. Case Management links consumers to all available and relevant services and helps them advocate for themselves whenever necessary and possible. The majority of services are provided with and by clients in their own homes or community settings. The Case Management staff is fluent in English, Polish and Spanish.

Next Steps services provide flexible, intensive long-term case management to consumers transitioning from hospitalization or homelessness and living in public housing in New Britain and Bristol. The program helps consumers learn what is needed to successfully manage symptoms and obtain entitlements, education, and employment. The majority of services are provided with and by clients in their own homes or community settings.

Supported Housing

The Supported Housing program provides long-term intensive case management assistance to help consumers stay in their housing of choice. The program aids individuals who are homeless, have a mental illness/addiction, or who have HIV/AIDS and are willing to participate in professional care. Staff provides liaison support with landlords, entitlement offices, health care, and other community stakeholders. Languages spoken include English, Jamaican Patois and Spanish, and towns of service include New Britain, Bristol, Plainville, Southington, Plymouth, Berlin and Burlington.

Community Outreach Program for Engagement (COPE)

The Community Outreach Program for Engagement (COPE) provides a comprehensive array of behavioral health services and substance abuse services to men and women over the age of 18.  Individual therapy, case management, and outreach modalities are utilized in accordance with an individualized treatment plan provided by the treatment team and the client.  Psychiatric assessment and medication management are also provided.  Services are provided in English and Spanish. 

The COPE program is a community based program designed for adults that are currently homeless, at risk of being homeless, or cannot attend traditional outpatient services for other reasons.  Treatment often focuses on issues including trauma, serious losses, mental health, and substance abuse, which directly impact their housing and financial status.  There is emphasis on services provided in the community, especially during the engagement phase, with case management services to assist with basic needs.  Funding is partially provided by the Department of Mental Health and Addiction Services (DMHAS).

The Community Support Program (CSP) provides innovative, recovery-oriented mental health and substance abuse rehabilitation services to assist individuals in achieving and maintaining independent community living. The services are delivered through a team approach which provides all-encompassing support for a variety of issues, including managing mental health or co-occurring disorders.

Days and Hours of Service:
Monday 8:00 am-8:00 pm; Tuesday-Thursday 8:00 am-6:00 pm; Friday 8:00 am-5:00 pm; Saturday 9:00 am-1:00 pm.

Community Connections for Survivors (CCFS)

Community Connections for Survivors provides wrap around services to children and adults who have experienced victimization from crime (physical, sexual, or verbal abuse, etc.), including unreported crimes. The program provides a unique approach to community based treatment to meet the needs of individuals who are in need of advocacy, linkage to services, and emotional support.  Referrals may be made through the Office of Victim Services, court personnel, hospitals, clinical providers, state agencies, or self referrals. Service areas include cities and towns of the following courts: Bristol, New Britain, Torrington, and Waterbury.
